Project Description

This project explores how a Community Health Worker (CHW) and Promotores program could improve health and connection across the Sierra Region.

CHWs and Promotores are trusted community members who help people understand their health options, connect to services, navigate complex systems, and share information in ways that feel familiar and culturally relevant.

Often CHWs and Promotores come from the same communities they serve, which helps build trust and reduce barriers to care. By listening to residents, local organizations, and partner agencies, the study aims to help shape a program that reflects the region’s culture, geography, and strengths.

What we learn will guide future investments in local residents who can offer one-on-one support, help people access important health resources, and ensure rural and historically disinvested communities are seen and heard.

With the right approach, a regional CHW/Promotores program can reduce isolation, improve access to care, and create a more connected and equitable health system across the Sierra Region.
